Abstract

Companion diagnostics (CDx) have become essential in precision oncology, enabling the identification of patients most likely to benefit from targeted therapies while minimizing unnecessary treatment. This seminar explores the integrated co-development of therapeutic agents and molecular diagnostics, emphasizing the role of next-generation sequencing (NGS) and digital PCR (dPCR) in enhancing predictive accuracy and clinical decision-making. Key topics include regulatory and clinical development frameworks, analytical validation of biomarker assays, and the translation of genomic insights into actionable treatment strategies. The discussion highlights how NGS offers broad genomic profiling for patient stratification, while dPCR provides ultra-sensitive, quantitative detection for residual disease monitoring and resistance mutation tracking. The seminar aims to provide an updated perspective on the evolving landscape of CDx development, the challenges of aligning assay design with drug development timelines, and the opportunities for implementing these technologies in routine clinical practice to advance personalized cancer care.

Next-generation sequencing (NGS) has become integral to molecular diagnostics in different human cancers, which enables comprehensive detection of actionable mutations that guide targeted therapy. This presentation focuses on the implementation of NGS as a companion diagnostic approach in routine testing of non-small cell lung cancer (NSCLC). Clinically relevant alterations in NSCLC driver genes such as EGFR, ALK, ROS1, and others, will be highlighted in the context of targeted therapies. Key components of the NGS workflow including pre-analytical sample assessment, assay design, sequencing, and result interpretation will be outlined. Practical challenges in routine laboratory settings, including sample quality, turnaround time, and quality assurance, will also be addressed. Through selected examples, how NGS-based companion diagnostics enable efficient identification of actionable mutations and support timely, personalized treatment strategies in lung cancer care will be demonstrated.
